By signing up to become an MFT Charity Champion, you agree to the following:

Welcome Packs

• After signing up and attending one of our ‘Come and Find Out More’ sessions, your Charity Champion welcome pack will be ready for you to collect.
• The first 30 Charity Champions to sign up and attend a ‘Come and Find Out More’ session will receive a welcome pack including a complimentary gift and refreshments for you and your team. These are available on a first-come, first-served basis.
• We will contact you to arrange collection of your welcome pack from the MFT Charity office at Cobbett House. If you are unable to collect it, please let us know.
• All Charity Champions who complete the sign-up process and attend a ‘Come and Find Out More’ session will receive a welcome pack. The complimentary gift and refreshments are limited to the first 30 Champions only.

Champion Role

• As a Charity Champion, we ask that you share relevant MFT Charity news and updates with your team and signpost grateful patients and families to MFT Charity where appropriate.
• Please let us know if you leave the Trust or are no longer able to continue as a Charity Champion, so we can keep our records up to date.
• We ask that you use any Charity Champion materials and MFT Charity branding appropriately and in line with any guidance provided.
• The Charity Champion role is voluntary and is intended to complement your existing role within the Trust.
